Signing up for home care services with Assisted Family Care is easy. You make one simple call, and then, we begin by reviewing your needs through a FREE in home assessment that includes discussions with involved family members. When needed, we will also consult with the physician(s) of the care recipient, social workers, hospitals, and assisted living or nursing home staff.

Next, we develop an individualized program to manage your daily needs based on the admission assessment. We work hard to match your needs and personality to the best possible caregiver. Of course, we want you to be a part of that selection process by letting us know whether our caregiver meets your needs and satisfaction.

After the care companion has been placed in your home, we continue in our unique system of care delivery through ongoing personalized contacts through telephone check-in, home visits, and more. Most importantly, we continually communicate with the caregiver who is providing care to you and track your progress.

Meet our team of professionals.

All of our caregivers have an understanding and compassion for others, those with disabilities, and for the challenges that continuous care can place on their families and friends. That is why one of the primary goals is to earn your trust and constantly remain sensitive to your needs as a whole family.

You can confidently rely on our staff because, each caregiver has prior hands-on experience in a healthcare setting, and all of our staff have been carefully screened and tested; meeting the highest standards of their profession. This screening includes a criminal background check, abuse registry and drug testing PRIOR to allowing them to work with your loved one. Your trust and safety are very important to us.
