What Is Home Care?

Home Care Explanation

The home care provided by Assisted Family Care is professional, person centered and focused on helping individuals who need assistance with activities of daily living (ADLs). This kind of home care is provided as a helpmate and support to the clinical care and directions given by the care client’s doctor or hospital. These non-medical services include things such as; bathing, dressing, grooming, light housekeeping, companionship, continence care, medication and care plan reminders, errands, escort and transportation services, assistance with meal planning, meal preparation, and much, much more. Our services are designed to be both proactive with targeted care client goals for improved health, and preventative in nature. We will work closely with you and your designated healthcare professionals to keep you healthy and independent. Whether you need us for a few hours a week, or around the clock 24-hour care…we’re here to fill that need.

Home Care can benefit you or your loved one in many ways:

  • Provides quicker recovery times – care clients recover quicker at home.
  • Provides encouragement – assists care clients in complying with their doctor’s or hospital’s instructions.
  • Can decrease the risk of re-hospitalizations and healthcare setbacks.
  • Provides privacy and dignity – care clients are in their own homes rather than an office or exam room.
  • Provides comfort – care clients are in a familiar and safe environment.
  • Cost effective – Home healthcare is usually less expensive compared to a long term care facility or a nursing home stay.
  • Peace of mind – knowing your loved one is being cared for by a trained and compassionate professional.
  • Provides added assurance that medications are taken properly through medication reminders.
  • Promotes wellness.

Plan of Care

Based on the admission assessment, an AFC professional staff member will develop an individualized plan of care for each care client’s needs. AFC will work closely with the care client’s physician, and maintain ongoing communication regarding the care client’s care and status. The doctor will be notified promptly of any changes or concerns.

We want you happy and well, therefore; our plan of care is built around your recommendations, desires and needs. But, also includes careful attention to your physician’s recommendations, and our observations. The choice in care is always the care client’s, and we assist care client’s in making wise and informed decisions to accomplish the goal of providing the right care, for the right reasons, and delivered the right way. We want what is best for you! We educate and empower care clients to make wise and healthy home care service choices, enabling them to stay healthier longer, or at least maximize their quality of life.
