This kind of home care is provided as a helpmate and support to the clinical care and directions given by the care client’s doctor or hospital. These non-medical services include things such as; bathing, dressing, grooming, light housekeeping, companionship, continence care, medication and care plan reminders, errands, escort and transportation services, assistance with meal planning, meal preparation, and much, much more. Our services are designed to be both proactive with targeted care client goals for improved health, and preventative in nature. Plan of Care

Based on the admission assessment, an AFC professional staff member will develop an individualized plan of care for each care client’s needs. AFC will work closely with the care client’s physician, and maintain ongoing communication regarding the care client’s care and status. The doctor will be notified promptly of any changes or concerns.
